The scholar’s ruling behind this
Permitted where the condition is determinate and its occurrence ascertainable, conditional on the declaration naming the condition precisely and on nothing being treated as endowed until it occurs.
Suspension (ta'liq) is accepted without dispute where the condition is death, which is the testamentary case. Wider suspension is contested, because a dedication that may never take effect leaves property in an uncertain state for an indefinite period. The permission recorded here rests on the condition being determinate and its occurrence ascertainable, so that the moment the dedication takes effect is knowable rather than arguable after the fact.

The ruling sets a condition and the platform records it. It does not yet test whether a waqf made under this form meets it, so seeing the form on offer does not mean the condition has been met. Until that test exists the obligation sits with whoever administers the waqf, which is where the ruling puts it in any case.
Determinacy of the condition is stated in this ruling and is not enforced by this adoption. No condition is bound to it. The platform does hold the suspended constitution mode structurally, so a declaration under this form will not be treated as constituted before its condition is recorded as met.
