The scholar’s ruling behind this
Permitted, conditional on a charitable remainder that takes the whole benefit on failure of the family line.
The family waqf is established practice and the majority permit it. The charitable remainder is what keeps a dedication from becoming a perpetual private settlement, and the permission recorded here rests on that remainder being present from the outset rather than added later.

This condition is recorded, not enforced
The ruling sets a condition and the platform records it. It does not yet test whether a waqf made under this form meets it, so seeing the form on offer does not mean the condition has been met. Until that test exists the obligation sits with whoever administers the waqf, which is where the ruling puts it in any case.
The charitable remainder is stated in this ruling and is not enforced by the platform. No condition is bound to this adoption, so the platform does not hold a waqf declared under this form to the remainder.
